Transitioning into Adulthood

As a young individual progressing towards adulthood, your journey through the Special Educational Needs and Disabilities (SEND) system involves a phase commonly referred to by professionals as 'Preparation For Adulthood' (PFA). With appropriate support, most young people with SEND can lead fulfilling and independent lives. They can secure employment, pursue education, and actively participate in social activities.


It is very important that, as a Young Person with SEND, you play your part in the development of your own future but remember - you need to be realistic in your approach!

If you have an Education, Health & Care Plan (EHCP) then, starting from Year 9, this must incorporate transition planning into all Annual Reviews

During this process, professionals will introduce you to PFA. They will provide you with valuable information and resources to assist you in making crucial decisions in various areas, including:

  • Employment - Navigating the job-seeking process.
  • Independent Living - Developing decision-making skills, exploring living arrangements, be it living alone or in supported housing options.
  • Community Inclusion - Engaging with the community, joining youth clubs, and participating in social groups.
  • Health - Addressing both mental and physical health concerns and wellbeing.
  • Money - Exploring grants, benefits, and financial assistance.
  • Education - Considering college and further education opportunities.
