As part of the EHC Journey, every Plan needs an Annual Review to take place each year. Starting from Year 9, these reviews must incorporate planning for a Young Person's transition to adulthood. The review should identify the Adult Social Care and Health support required as a Young Person approaches the age of 18.
Not all individuals with an EHCP will stay in formal education until the age of 25, most will make the transition into adulthood at 18. Some may choose to go into immediate employment, others may opt for higher education or receive full support from Health or Social Care services.
The EHCP Annual Review should be informed by ongoing transition planning and cover discussions and support for the following:
- Helping Young People make decisions for themselves.
- Planning appropriate Post-16 educational pathways leading to higher education or employment.
- Access to Health and Social Care services.
- Understanding what universal services can offer support.
- Where to find assistance, information, advice and guidance from various sources.
- The future living arrangements the Young Person desires.
- Comprehensive planning for transitioning to full Health and Social Care services once an EHCP is ceased.
- Support for finding employment and succeeding in a job.
- Understanding welfare and housing benefits.
Each EHCP transition plan is tailored to the individual and their post-school or college pathway, which may involve higher education, employment, or full adult health or care services where their future needs will be met by Social Care or health agencies. The plan should also outline arrangements for when their EHCP will cease.
Professionals supporting young people with PFA should focus on their aspirations, ambitions, and providing support for greater independence and employability.
