Who is the service for?
All students in Year 10 and above (approximately 14+ years old) up to a potential 25 years old with an Education Health and Care plan.

Preparing for Adulthood means preparing for:
• Higher education and/or employment – this includes exploring different employment options, such as support for becoming self-employed and help from supported employment agencies
• Independent living – this means young people having choice, control and freedom over their lives and the support they have, their accommodation and living arrangements, including supported living
• Participating in society, including having friends and supportive relationships, and participating in, and contributing to, the local community
• Being as healthy as possible in adult life
What we do
The team supports young people through the final stages of education to Adulthood pathways.
We also work with all settings that provide an education to this age group
schools, colleges, training providers, or employers supporting apprentices (private, voluntary and independent) in the London Borough of Hounslow and surrounding areas.
Our role is to support the inclusion of students with recognised Special Educational Needs and/or Disability (SEND) who still hold a current Education, Health and Care Plan (EHCP).
Possible Adulthood pathways include:
• Social Care packages of support for young people who are unlikely to ever be economically active, live independently or require high levels of support with their physical or mental health, or accessing the community
• Supported internship, traineeships or apprenticeships to support a young person to be ‘work ready’
• Moving from education into full time employment
• Moving into higher education at Level 4 or above
The team will also cease the Education, Health and Care Plan when the student has reached the end of education and is prepared to start their planned adulthood pathway
How it works
The team will contact the parent/carer of every student with an EHCP every year from Year 10 until a student’s EHCP is ready to cease.
This is to check students are making progress through KS4 and KS5 and any training opportunities for their personal pathway destination.
Sometimes this will include working with the wider network around a young person to ensure there is an appropriate transition into adult services.
Ceasing an EHCP is recognition that a student has achieved their personal maximum in the formal education system; it is not the end of education, but the start of adulthood.
Students with Education, Health and Care Plans will be automatically be transferred into the Preparation for Adulthood Team over the summer holiday between Year 9 and 10.
Each student will have a named case worker.
